It is quite an obvious combo, however it is very weak. The first reason is, it can be interrupted. You can't press the buttons within .1 of one another. I've tried it offline, to remove lag from the equation. My computer is quite powerful, I've never experienced lag in the game except when I minimize and maximize it a couple items in a row. Anyways, when I push HoL and Defiance as fast as I can, only HoL activates - you have to wait a moment between the two items. Thus, it can be easily negated by the attacks in a fight - a rooks archer shoulder, a generals minions, spit or ooze or mist(though it can be timed if experiencing spit or ooze or mist) and any rapid damage, especially the late game super fast normal attacks. Not only that, this combo costs as much as a mageslayer, which gives awesome attack power and life leech, or the other 8k artifact, which means 25% att speed and 24 hp/sec - a formiddable healing that is guaranteed.
