IMO, a premade has two or more of the following criteria:
- Players are on voice-chat and communicate openly and often throughout the game
- Players have a discussed and/or practiced team synergy
- Players are competitive in nature, with concerns about not only winning the current match, but winning more matches than the competitive premade next door
- Players are elitist and think that any PuG they beat simply has that much less skill
The first two are the most important criteria, while the third and fourth are simply stereotyping. If some of the criteria are fulfilled, one cannot assume that they all must hold true.
edit: in contrast to the two posts above mine, I do not think that "playing with friends" necessarily means it's a premade, though from the outside perspective it may be difficult to differentiate
