Are you referring to using IconPackager?
If so, you would normally do the following to restore and/or repair:
1 - Start IconPackager > Tools > Restore all Icons > "Yes" to confirm > "OK" to apply.
2 - Repeat #1 for the Rebuild Icon Cache, Repair Icon Images, and Repair Shell Icons
3 - Reboot computer
If these fail for some reason, and you are running Windows XP - you can also unload WindowBlinds if you are running it, right-click the desktop > select Properties > click the Themes tab > select Windows XP in the Theme drop-down list > click Apply.
Note - if the apply button is not available, you can select Windows Classic, and then reselect Windows XP to activate the Apply button.
